Retirement of the Exemplar Allergy Practice

We regret to inform you that Exemplar Allergy will be retiring its practice effective Friday, June 13, 2025. This decision was not made lightly, as we have been honored to serve you and our communities in Harrison and Marion counties over the last 3 decades.

To ensure a smooth transition, please contact your primary care physician or a new allergy specialist at WVU at 855-WVU-CARE (855-988-2273) or

Obtain your medical records: Send us your request via email to: ExemplarMedicalRequests@gmail.com and include your first and last name, you mobile phone and email. You will be receiving an email invitation from RXNT.com (the medical platform that Exemplar has used for managing of your electronic medical records).  PLEASE make sure to check your email for spam so that you don't miss the invitation to set up your account, for access to download your records.  PLEASE NOTE that your password/pin to set up your account only lasts for 72 hours once sent to your email.

 

If you are currently on allergy shots, please contact us immediately to pick up your vial(s) for your new provider.   

 

You may reach our office at (304) 848-2400 until June 23, 2025, to arrange for record transfers or address any questions.
